How to treat nodular scabies

Dear Doctor

It start only scrotum is itchy then I used ring guard but it didn’t work.

After that some bumps came on the scrotum, itching very high after that I used caladryl and took amoxicillin 625 mg and after 3-4 days some bumps comes on penis upper skin and scrotum and penis. Both are itching very high and skin is full dry.

Ring Guard Cream

Ring Guard is a cream which contains the antifungal Miconazole and the antibiotic Neomycin.

Caladryl

Caladryl is a lotion which contains the antihistamine Diphenhydramine and Calamine which dries oozing lesions. It is used to reduce the itch and pain caused by poison ivy, poison oak and poison sumac as well as insect bites, sunburns and minor cuts.

 

Nodular Scabies Treatment

Your symptoms are suggestive of nodular scabies.

Medications used to treat nodular scabies include:

Permethrin cream

Scabies is treated by applying Permethrin 5% cream which contains an insecticide that kills the scabies mite. Permethrin cream is applied from the neck to the toes and washed off after 24 hours. The application is repeated after 7 days to ensure that any mites that may have hatched from eggs are killed.

Possible side effects of permethrin cream include skin irritation, stinging, numbness and tingling sensations.

 

Ivermectin

Ivermectin can be taken by mouth at a dose of 200 micrograms/kg body weight and repeated after 2 weeks. Pregnant and nursing women should not use this medication.

 

Oral antihistamines

Antihistamines can be taken to control the itch. Examples include the non-sedating fexofenadine (Allegra) and loratadine (Claritin) as well as the less sedating cetirizine (Zyrtec). The sedating diphenhydramine (Benadryl) and hydroxyzine (Atarax, Vistaril) are also used to reduce the itch especially if it disturbs sleep.

 

Topical steroids

Steroid creams like 1% Hydrocortisone are used for nodular scabies to flatten the bumps. Steroids can also be injected into the nodules to soften and smoothen them.

 

Tacrolimus ointment

Tacrolimus 0.03% ointment applied twice daily for 2 weeks has been used successfully to treat patients who have persistent itchy nodules after applying permethrin cream.
